2024 Public Health Improvement Plan (PHIP)

and

2023 Community Health Assessment (CHA)


The San Luis Valley Public Health Partnership (SLV PHP) is excited to share both their 2024-2029 Public Health Improvement Plan (PHIP) and their 2023 SLV Community Health Assessment (CHA).

The PHIP is the culmination of over two years of collaborative data collection, analysis, and planning led by the six public health directors in the San Luis Valley. The CHA is a series of assessments conducted to help describe the needs, issues, and health status of a community. After reviewing data from the 2023 CHA, along with examining capacity, funding, community partnerships, and current work, goals were set for SLV local public health agencies.

The SLV PHP identified three priorities to focus on over the next five years (through 2029):

  • Substance Use/Misuse & Mental Health

  • Prevention & Education

  • Services & Support for Older Adults

In Colorado, this process is required every five years by all governmental public health agencies, per the Public Health Act of 2008. the overarching goal of these efforts is to increase the availability and quality of public health services and ultimately to improve health outcomes.

The San Luis Valley Public Health Partnership pools resources and works together on a regional level to complete these assessments.
